Circumscribed adnexial tumor, with basaloid aggregates and nice hidden shadow cells: PILOMATRICOMA. 

The patient seems too elderly for this type of pathology, but Malherbe's tumor has been described on elderly people, too.

I'm not sure about this tumor. There are suggestions of matrical differentiation, namely, aggregates of basaloid cells associated to pale areas with shadow cells-like features, but some of these pale areas represent a rather hyalinized stroma with spindled cells. On the other hand, neoplastic lobules show peripheral palisading with retraction artifact. Basal cell carcinoma with matrical differentiation may be a point.

I have thought also of BCC with matrical differentiation.

I thought this was either a BCC with metrical differentiation or malignant pilomatricoma.
